If you need to enroll your child(ren) , please go to our Welcome Center located at 4420 Manchester Rd. or you may call them at 513/420-4561.  The Welcome Center operating hours are from 8: 00 A. M. until 3:00 P. M. Accommodations after 3:00 P. M. will be arranged by appointment. After Aug. 2 if your child needs to enroll at Mayfield and they are coming from another elementary school here in the Middletown City School district, you may enroll at Mayfield. There is no need to go to the Welcome Center.

You will need to have the following information for your child in order to enroll him / her in school.

Birth Certificate
Shot Record
Proof of Address
Picture ID of parent or guardian
If needed, custody papers
Address of previous school

Sep. 9 - PTO Mtg. - 4:00 PM
Sep. 15 - Early Release at 2:45 PM
Sep. 16 - Interim Reports Come Home
Sep. 21 - Skating Party - 6:30-8:30PM
Sep. 30 - Picture Day
